Synonyms share meaning but rarely connotation. Choose the one that fits the register and the surrounding sentence, not simply the closest dictionary match — which is why the "most correct" answer is the one that would not change how the sentence feels.
Try 4 short questions throughout this guide. Check each answer to learn from the explanation.

Denotation and connotation
Denotation is what a word means; connotation is what it makes the reader feel. Thin, slender, slim, skinny and emaciated all denote low body weight, and no two of them carry the same attitude. Two are neutral, one is complimentary, one is unkind, and one is clinical.
That is why synonym questions rarely have two defensible answers. Several options will match the meaning; only one will match the meaning and the tone of the sentence around it.
The same meaning, different weight
Neutral
Positive
Negative
determined
resolute
stubborn
thrifty
economical
miserly
confident
assured
arrogant
inexpensive
affordable
cheap
unusual
distinctive
odd
inquisitive
curious
nosy
Register matters as much as connotation. In a clinical note, the patient was upset and the patient was distraught are not interchangeable, and neither would be replaced by the patient was gutted. Match the formality of the writing you are given.
Question 1 of 4
A patient information leaflet needs a word for "not expensive". Which fits best?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. affordable
Affordable is neutral-to-positive and suits a leaflet addressing patients directly. Cheap carries a suggestion of poor quality, while miserly describes a person and paltry dismisses something as inadequate.
Using context to choose
The sentence usually tells you which shade is wanted, if you read for it.
Look for a contrast — a "but" or "however" often signals that the missing word opposes something nearby.
Look for the writer's stance. If the passage is sympathetic, an unkind synonym cannot be right.
Look at who is speaking and to whom. A patient leaflet and a research abstract call for different words.
Substitute your choice back into the sentence and read the whole thing. A word that changes the meaning of the sentence around it is the wrong one.
Question 2 of 4
Why can two words be synonyms and still not be interchangeable?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. They may share denotation while carrying different connotations or belonging to different registers.
Shared meaning does not mean shared feeling, and it does not mean shared formality — slim and skinny illustrate the first, upset and gutted the second. Both words in such a pair are perfectly grammatical; the difference is in what they convey.
Choosing by connotation
Worked example
Which word best completes the sentence? Despite the risks, she remained ____ in her decision to complete the course. Options: stubborn, resolute, obstinate, inflexible.
All four denote refusing to change one's mind, so meaning alone will not decide it.
Read the stance: "despite the risks" frames her as persisting through difficulty, which is sympathetic.
Sort the options by connotation: stubborn, obstinate and inflexible are all critical.
Only one is admiring.
Resolute. The sentence presents her persistence approvingly, so the synonym has to be the approving one — a correct denotation with the wrong connotation is still a wrong answer.

What the HESI asks most
The same core ideas appear in different wording. If you can answer these without stopping to think, you have what this section requires.
Which word is closest in meaning to the underlined word?
Substitute each option into the sentence. The right one leaves the meaning and tone unchanged.
Which word best completes the sentence?
Read the writer's stance and match the connotation, not just the definition.
Which of these words is most formal?
A register question. Match the formality of the surrounding writing.
Question 3 of 4
Which word best completes the sentence? Despite the difficulty of the placement, she remained ____ about finishing it.
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. determined
"Despite the difficulty" frames her persistence sympathetically, and determined is the neutral-to-positive option. The other three all denote much the same persistence while criticising it, which clashes with the sentence's stance.
Key points
Synonyms share denotation and rarely share connotation.
Several options will match the meaning; only one matches the tone.
Read the writer's stance to work out which shade is wanted.
Match register: clinical, academic and conversational words are not interchangeable.
Substitute your choice back into the whole sentence before committing.
Question 4 of 4
Which word is closest in meaning to alleviate in: The medication alleviated her symptoms within an hour.?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. eased
To alleviate is to make less severe, which eased matches exactly. Eliminated overstates it by implying the symptoms went entirely, concealed suggests hiding rather than reducing, and aggravated means the opposite.
