November 2026 - Determining a Sequence of Events Study Guide for nursing students in
Sequence questions ask what happened first, next or last. The order of events is often not the order of the sentences, so the signal words — before, after, meanwhile, subsequently, finally — carry the answer rather than the layout of the paragraph.

The signal words that carry order
Sequence is almost always marked. Learn the four groups and you can rebuild a timeline without rereading.
Group
Words
What they tell you
Beginning
first, initially, to begin, at the outset
The starting point
Middle
then, next, after, once, subsequently, following
One step follows another
Simultaneous
meanwhile, while, at the same time, during
Two things overlap — neither is "next"
End
finally, last, eventually, in the end
The closing step
Why sentence order is not event order
Writers reorder events for effect all the time. A narrative passage may open at the crisis and then explain how it came about. A procedural passage may state the outcome first, then the steps. And a single sentence can carry two events in reverse: "before checking the dressing, she washed her hands" puts the washing first in time and second in the sentence.
The safe method is to build a short timeline as you read, in event order rather than reading order, and then answer from the timeline.
Watch "before". It is the one signal word that reverses the sentence: whatever follows "before" happens later, and the other clause happens first. Questions about procedures are built on this constantly, because it is where careful readers still slip.
Question 1 of 4
Reading Passage
The audit began after a near miss on the surgical ward. A patient had been given a double dose of an anticoagulant because two nurses each believed the other had not yet administered it. In the weeks that followed, the trust introduced a signing sheet at the bedside. Before the sheet arrived, however, the ward had already changed its handover to include a spoken medication check, which the ward manager had proposed on the morning after the incident. The audit's final report, published four months later, credited the improvement mainly to the handover change.
Which sentence establishes that the handover change came before the signing sheet?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: C. "Before the sheet arrived, however, the ward had already changed its handover …"
"Before the sheet arrived … had already changed" gives the order outright, and it is the sentence that reverses the reading order. The first fixes the audit relative to the incident, the second introduces the sheet without ordering it against the handover, and the fourth only dates the report.
Rebuilding a timeline from out-of-order sentences
Worked example
Passage: "By the time the ambulance arrived, the bleeding had been controlled. A colleague had applied direct pressure while the ward clerk called for help. Before either of those things, the patient had been helped back into bed."
List events in the order the sentences give them: ambulance arrives, pressure applied and call made, patient helped into bed.
Read the signals. "By the time" means the control came first. "While" means pressure and the call overlapped. "Before either of those things" puts the bed first.
Rebuild in event order: helped into bed, then pressure and the call together, then the ambulance arrives.
Note that the sentence order is exactly the reverse of the event order — which is the point of the question.
Helped into bed → direct pressure applied and help called (at the same time) → ambulance arrives. The paragraph tells it backwards.
Question 2 of 4
Reading Passage
The audit began after a near miss on the surgical ward. A patient had been given a double dose of an anticoagulant because two nurses each believed the other had not yet administered it. In the weeks that followed, the trust introduced a signing sheet at the bedside. Before the sheet arrived, however, the ward had already changed its handover to include a spoken medication check, which the ward manager had proposed on the morning after the incident. The audit's final report, published four months later, credited the improvement mainly to the handover change.
Which of these happened first?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. The patient received a double dose.
The double dose is the near miss that everything else responds to: the audit begins "after" it, the manager's proposal comes "on the morning after the incident", and the sheet arrives later still. The passage opens with the audit, which is why sentence order is the trap here.
The traps
Sentence order — a choice that lists events in the order they appear on the page.
A "meanwhile" treated as "next" — two overlapping events placed in sequence.
A flashback taken as the present — background explanation read as the next step.
A step invented from plausibility — the obvious order in real life, where the passage gives a different one.
Terms to know
Chronological order
Events arranged by when they happened.
Signal word
A word marking order or overlap: before, after, meanwhile, finally.
Flashback
A passage that moves back in time to explain something already described.
Simultaneous
Happening at the same time. Neither event follows the other.
Procedural text
A text describing steps to be carried out in order, where sequence is the content.
What the TEAS asks most
The same core ideas appear in different wording. If you can answer these without stopping to think, you have what this section requires.
Which event happened first?
Answer from a timeline built on signal words, not from which sentence came first.
What happened immediately after X?
Find X on your timeline and take the next distinct step — not an event described as simultaneous with it.
Put the steps of the procedure in order.
Work through the passage marking each step, and re-read any sentence containing "before" twice.
Question 3 of 4
Reading Passage
The audit began after a near miss on the surgical ward. A patient had been given a double dose of an anticoagulant because two nurses each believed the other had not yet administered it. In the weeks that followed, the trust introduced a signing sheet at the bedside. Before the sheet arrived, however, the ward had already changed its handover to include a spoken medication check, which the ward manager had proposed on the morning after the incident. The audit's final report, published four months later, credited the improvement mainly to the handover change.
According to the passage, what happened immediately after the incident?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. The ward manager proposed a spoken medication check.
The proposal is dated to "the morning after the incident", making it the first response described. The passage explicitly places the signing sheet after the handover change, the report comes four months later, and no interviews are mentioned anywhere.
Key points
Event order and sentence order are different things; questions test the difference.
Signal words carry the sequence. Find them before you decide anything.
"Before" reverses its sentence: the later event follows the word.
"Meanwhile" and "while" mean overlap, so neither event is "next".
Build a short timeline as you read and answer from it, not from memory of the paragraph.
Question 4 of 4
Reading Passage
The audit began after a near miss on the surgical ward. A patient had been given a double dose of an anticoagulant because two nurses each believed the other had not yet administered it. In the weeks that followed, the trust introduced a signing sheet at the bedside. Before the sheet arrived, however, the ward had already changed its handover to include a spoken medication check, which the ward manager had proposed on the morning after the incident. The audit's final report, published four months later, credited the improvement mainly to the handover change.
Which statement about the sequence is inaccurate?
Choose one answer, then check it.
View the Answer and Explanation
Correct answer: B. The signing sheet was introduced before the handover changed.
That reverses the order the passage gives, since the handover change was already in place before the sheet arrived. The other three match the timeline: the report closes it, the audit follows the near miss, and the proposal precedes the sheet.
